DICOM tags can be transmitted to a modality through a DICOM header. This header contains a set of predefined DICOM tags that describe the attributes of the data being transmitted. These tags include information such as patient demographics, imaging modality, date of acquisition, and study description.

When a modality receives this header, it uses the information contained within the DICOM tags to generate a DICOM file. The file is then transmitted back to the sending device or stored in a DICOM server for later retrieval.

DICOM tags can also be manipulated or added to after the image is acquired in order to provide additional information. This process can be accomplished using DICOM software such as PACS viewers or DICOM editing software.
