To obtain underline annotations when editing text that already contains them in PDF.js, follow these steps:

1. Open the PDF file that contains the text with underline annotations in PDF.js.
2. Click on the "Edit" button on the toolbar.
3. Navigate to the page where the text with underline annotations is located and click on the text to be edited.
4. Use the cursor keys to move to the underline annotation and select it.
5. Click on the annotation to open the Annotation Properties dialog.
6. In the Properties dialog, select the "Underline" option from the "Type" dropdown menu.
